Thank you for considering me as a doctor for yourself or your child!

Payments

This is an out-of-network practice meaning that we do not participate with insurance plans, and we have opted out of Medicare. We do not accept insurance as a form of payment.

Also, we do not participate in a single case agreements with any of the insurance careers. You may want to check with your insurance career If your insurance plan provides out-of-network mental health coverage and if you may be able to submit super bills to receive reimbursement for a portion of the fees that you have paid for my services. Reimbursement would typically be made by insurance directly to you once you have met your deductible. We will provide you super bills for all visits and appointments.

We accept check, and all major credit cards as forms of payment. Payment is due at the time services are rendered.

Initial Consultation

We require a non-refunduble deposit at the time of booking for us to reserve your initial appointment. The amount of the deposit will be applied toward the total cost of your first visit.

Your deposit will remain valid for up 3 months from your original appointment date.

This policy is to encourage patient accountability for their scheduled appointments since no-shows limit other patients the opportunity to secure and receive care.

Please note that payment of the deposit and initial appointment fees do not represent a doctor-patient relationship, and it is advised to follow through with the doctor’s treatment plan.

Cancellation Policy

If you need to cancel or for some reason miss your appointment we ask to contact us at least 2-business days in advance to avoid getting charged the full amount for the visit.

Credit card information/authorization is required to keep on file.

Please note that we do not provide a super bill for missed appointments

Following your treatment plan (attending all scheduled visits as recommended) is important for maintaining effective and consistent care.

Our Patient Portal:

We use HIPAA-protected, secure and user-friendly portal “Simple Practice” for secure communications (messaging) between you and your doctor. Once registered, you will be able to stay connected with us and have a quick access to your doctor.

Good Faith Estimates:

As a part of No Surprise Act policy you have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services.

You can ask your health care provider, for a Good Faith Estimate at least 1 business day before you schedule a service.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
