Payments

This is an out-of-network practice meaning that I do not participate with insurance plans, and I opted out of Medicare.

Please note, that I do not accept insurance as a form of payment.

Also, I do not participate in single case agreements with any of the insurance careers.

You may want to check with your insurance carrier if your insurance plan provides out-of-network mental health coverage and if you may be able to submit super bills to receive reimbursement for a portion of the fees that you have paid for my services. Reimbursement would typically be made by insurance directly to you once you have met your deductible.

My office will provide you super bills for all attended visits.

I accepts checks, and all major credit cards as forms of payment. Payment is due at the time services are rendered.

Initial Consultation

A nonrefundable deposit will be required at the time of booking for us to reserve your initial appointment. The amount of the deposit will be applied toward the total cost of your first visit.

Your deposit will remain valid for up to 3 months from your original appointment date.

Please note that payment of the deposit and initial appointment fees do not represent a doctor-patient relationship.

Follow Up Appointments

Individual treatment plan regarding the frequency and duration of your follow up appointments will be discussed with each patient individually.

Following your treatment plan (attending all scheduled visits as recommended) is important for maintaining effective and consistent care.

Cancellation Policy

If you need to cancel or for some reason, miss your appointment please contact my office at least 2-business days in advance to avoid getting charged the full amount for the visit.

Please note that I do not provide a super bill for missed appointments.

Physician-Patient Portal:

I use the HIPAA-protected, secure, and user-friendly portal “Simple Practice” for secure communications (messaging) between you and your doctor. Once registered, you will be able to stay connected with me.

Good Faith Estimates:

As a part of No Surprise Act policy you have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services.

You can ask your health care provider, for a Good Faith Estimate at least 1 business day before you schedule a service.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
